A Headshot is a successful attack to the head of an enemy with a scoped Sniper Rifle, Huntsman arrow or Ambassador bullet. When successful, the attack will score a Critical hit and do additional damage.
Snipers may only perform headshots when they are scoped with the Sniper Rifle and have waited twenty-three milliseconds for a headshot to be considered a Critical. Headshots with the Ambassador can only score Critical hits when an accuracy dropoff period has dissipated (also known as "cool down" period), as indicated by a normal sized crosshair. Getting a headshot kill with the Sydney Sleeper will still produce the headshot icon, however, it will not grant a Critical hit and will do damage as if you had hit any other part of the body..
Players earn 1.5 points for getting a headshot kill; one point for the kill and the half point for the headshot itself. Scoring a headshot with a Huntsman arrow yields another half point in addition to the aforementioned bonus, resulting in 2 points per kill. A fully charged Sniper Rifle headshot results in an instant kill — even on a overhealed Heavy — unless a damage nullifying effect is active on the target.
Pyros with exceptional aim can score headshots by reflecting arrows with the compression blast.
It is important to note the hitboxes of the classes are not perfectly aligned to their onscreen appearance, especially while performing certain animations such as walking backwards or taunting. It is thus critical to adjust shots accordingly. It may be helpful to remember that the positions of the head hitboxes are centered on the axes of rotation, likely to prevent exploits.
