Loot Island is the island in the Underworld with the big blue skull on it. It is only reachable by jumping through the red portal after killing Monoculus. Once there you can climb up the steps and jump into the book to get the achievement Dive into a Good book. Which gives you the Bombinomicon ,a wearable Misc that grants you the ability to produce a purple explosion on death.
