At least one of the item creators made multiple posts over on the Facepunch forums saying that the item wasn't a reference to Doctor Who, and there was another post going over how the idea came from "Space Oddity" and Y: The Last Man Vol. 3. I believe that this hat is a reference to the Doctor Who episode "Silence in the Library" where the Vashta Nerada kill one of the crew and leaves his skeleton inside his body.
