I have many medals!
The Heavy

Miscellaneous items, commonly abbreviated as Misc. items, are wearable items with their own 'Misc' section in the loadout screen, though they count as headgear for crafting purposes. Like hats, they have no effect on gameplay and exist for cosmetic purposes only. As they occupy their own equipment slot, they can be worn in conjunction with hats. Misc. items are either unlocked through achieving certain requirements, or dropped during in-game playtime.

In total, there are 27 misc. items, with eight classes as of yet possessing their own specific items. The only class without a misc. being the Heavy.

Crafting

Main article: Crafting

Because most Misc. items are achievement linked or promotional items, they are generally incompatible with the crafting system. Most of them cannot be crafted into other items, or crafted themselves from other items. The exceptions are Misc. items that can be found in-game through the item drop system, which are treated exactly the same as headgear. As such, they can be crafted in any blueprint that requires headgear to function. These are the current Misc. items that function as headgear, in order of their addition to the game:

The Classless Update (August 13, 2009)

  • The first misc. item, the Soldier's Gentle Manne's Service Medal, is added to the game. The medal is awarded to the first 11,111 users who stumbled upon a hidden page on the TF2 Official Blog, with each medal featuring a unique number. The medal can no longer be obtained.

The WAR! Update (December 17, 2009)

  • The Camera Beard misc. item for the Spy becomes a misc. item, allowing it to be worn with other hats for the class. This increases its drop rate to that of normal weapons, resulting in a loss of its rare value and a significant increase in the amount of players that own it. However, its drop rate has been returned back to that of hats later on.

First Community Contribution Update (March 18, 2010)

  • The first set of community hats from Valve's Contribute! site are patched in, along with a new misc. item for the Medic, the Physician's Procedure Mask. The Camera Beard's drop rate is restored to the same drop rate as normal hats. Misc. items can now be used along with hats in the Rebuild Headgear recipe.

The 119th Update

  • The Grizzled Veteran, Soldier of Fortune and Mercenary medals are awarded to players depending on the date of their first play. They are the first misc. items that can be equipped by all classes. These medals are Gold, Silver, and Bronze in appearance respectively.

May 5, 2010

  • The Primeval Warrior medal is awarded to all players who participated in the original beta release of Team Fortress 2, with the medals of those eligible being changed accordingly. These medals are Platinum in appearance.

Second Community Contribution Update (May 21, 2010)

  • The Whiskered Gentleman misc. item for the Pyro is reassigned to the Misc. slot, so that it may be worn with other hats.

The Mac Update

  • The Earbuds are awarded to players who launched TF2 on a Mac between June 10, 2010 and August 16, 2010.

June 28, 2010

  • A Greentooth (Polycount icon) pin is revealed by Polycount as a misc item available to community members who submitted entries for the Polycount Pack contest. The item was contributed by Polycount member DKK and was expected to be released alongside the Polycount Pack. It was released well before, however.

The Engineer Update

  • A second Misc. slot is added, though no items have been released for the slot at the time.

August 9, 2010

Mann-Conomy Update

  • The second Misc. slot (MISC2) is removed and is replaced by the Action slot. Four badges are also made available by playing the Dueling Mini-Game and winning duels. The Polycount Pin is also added.
