Loot Island is the island in the Underworld section of Eyeaduct with the big blue skull on it. It is only reachable by jumping through the red portal after killing Monoculus. Once there, you can climb up the steps and jump into the book to get the achievement Dive Into a Good Book. Doing so awards you the Bombinomicon, a miscellaneous item that produces a purple explosion upon death when it is equipped.
