It's ring-shaped, it's gift-wrapped, it's basically useless, and it's really expensive ($100!). In short, it's the most accurate simulation of an actual Valentine's Day gift ever made available to the public.
The TF2 Team

The Something Special For Someone Special is a crafting item, appearing as a silver diamond ring, sitting in an open blue-violet colored case with a grey felt inside that is traditionally associated with an engagement ring.

When it shows up in your special someone's backpack, they can click on it to open a menu that will let them accept your proposal. Once the proposal's been accepted, a message will be broadcast to the entire community that will include your name, your special someone's name, and whatever you decided to call the ring.

The gift then turns into a matching pair of diamond band miscellaneous items which carries the name of each party, sender first, with the following description "A perpetual bond of commitment between (Sender) and (Recipient), forever and ever and ever. XoXoX "

When the ring is purchased from the store, you automatically receive free gift wrap and a name tag to use on the diamond, along with any other additional items for a purchase above the free item threshold. Note the diamond bands cannot be renamed using these tags afterwards.

The Something Special for Someone Special was contributed to the game via the Steam Workshop, under the name "Engagement Ring".
