The Quadwrangler is a promotional item. It is still unavailable on Team Fortress but was announced on the app page of Quantum Conundrum.
It is described as "This quantum-collared lab coat includes a handy rubber pipe that siphons blood from your heart, where it is not needed, to your shooting arm, where it is!"
