The Ghastlier Gibus is a Halloween Special Event item that has upgraded the Ghastly Gibus for those who earned it before Halloween 2010. The upgraded hat sports a new decoration. This item cannot be traded, however it can be used with the Name Tag, Description Tag, and Paint.
In the Backpack it carries a description of "Even more ghastly."
