The Mediterranean Mercenary is a community-created cosmetic item for the Heavy. It gives him a brown beard with a golden ring, a golden earring on his right ear, and brown hair tied into a braided ponytail with two golden rings that moves with the player.
This item has two styles, named "Normal" and "Defiant Dey". The "Defiant Dey" style gives him a shorter beard and longer hair, which is partially covered by a golden Turban helmet with a team-colored turban. Both styles give the Heavy a stereotypical look associated with North African cultures in the Mediterranean.
The Mediterranean Mercenary was contributed to the Steam Workshop.

Bugs
- Taunting with any primary weapon will remove the beard slightly from the Heavy's face. This also happens in the animation of the choosing class menu.
Trivia
- The "Defiant Dey" style refers to an honorific awarded to high-ranking officials in the Ottoman Empire.
- The two cities mentioned in the item's publicity blurb, Salé and Algiers, are North African cities close to the Mediterranean.
