The Spine Chilling Skull 2011 is a headwear item wearable by all classes. It is fashioned after the Spine-Chilling Skull, but it is slightly darker and has buck teeth instead of the Spine-Chilling Skull's pointed teeth. The Skull has 3 styles:
- Tingling, which has small horns positioned on the forehead area facing upward like a devil
- Cooling, similar to the Spine-Chilling Skull with curved ram horns, but the horns are slightly smaller
- Twisting, similar to the Spine-Chilling Skull, but the horns are larger and have a more prominent curve
The Spine-Chilling Skull 2011 hat was only available for purchase from the Mann Co. Store during the Very Scary Halloween Special event, which ran from October 27 until November 6, 2011.
There are also three separate but distinct versions of this hat, which each permanently take the form of just one of the three styles. All three variants are only available by crafting using the Fabricate Haunted Headgear recipe.

- When crafting using the Fabricate Haunted Headgear recipe, the player will receive a version of the Spine-Chilling Skull 2011 hat that only has one of the three possible styles. The style of the skull cannot be changed to the other styles. Only hats purchased through the Mann Co. Store have the option of switching between the three styles.

Trivia
- Bay Raitt mentioned the various styles of this item in the blog post Making of a Skull Hat.
- Unlike the original Spine-Chilling Skull, applying paint to this item will only change the color of the horns, and not affect the skull itself.
- There was a glitch on the 11th November where uncraftable hats became craftable after trading them. This means there will be a few truly clean Spine-Chilling Skull 2011.
