The buttons on the back are affected by the paint, but there's no good way to show it with a different rotation. One suggestion would be making two tabs, one for "Front" and other for "Back" (visible both on the paintable variants and the infobox). However, it's worth noting that these wouldn't be styles, but WOULD use the "style=" parameter anyway. Prize Plushy uses a different method to show the back that affects paintability, but I don't think it would look good for the Siberian Sophisticate. I have attempted in the past trying to get a pose similar to the Surgeon's Sidearms, Shogun's Shoulder Guard and Clue Hairdo but to no avail.

I think having the images combined into one would look better, it would let people see the whole thing at once too.

I think combining the two images into one would look completely awful. Unless you can show an example of what you're picturing or something I can't make it look acceptable in my head.
Well, we could do an approach similar to the Prize Plushy. But, I don't know any way that would look pleasing to look at. If we just use the heavy_shirt_materials but rotated from the backside, the image will be too wide and will appear small on the paint variants table. Although mentions about paintability on the item's summary hasn't been a thing for a long time, I think it would be important here, in case we combine the images and leave just the backside of the "puffy" part or whatever.
Increasing image size should be fine if it looks too small, though if I'm honest people can also click on the variants.

Originally, the File:Das_Feelinbeterbager.png infobox image showed both the front and back. I decided to remake it to show most of the item as possible, since the paint variants already showed everything being painted anyway, including the unpainted variants. It was somewhat wide, but it also didn't use very well the available empty space. But, I do think we could make a "Front" and "Back" buttons for the Painted variants (and on the infobox) not only for this item, but a couple others too. Cape-related items like the Starduster could benefit from it as well.
By including both the front and backside of Heavy's body_materials on the same image, I believe that the main thing that people would be interested while painting this item (the tie), would be too small to notice, and would require to click on every paint variants in order to increase. I don't think it would be the most ideal solution.
I am thinking of ways that it could be made similar to Prize Plushy, but I got nothing so far. Having a non-ref pose could work too, but I have attempted that in the past and didn't got a good looking one. Maybe a completely custom pose, made from scratch, meant to show both buttons and the tie on the same image (not 2 images into 1), could work. Though I wouldn't be able to do that as I never tried doing custom animations / poses before.
