This hat appears to be a skinned copy of the "Medic's Officer Hat" file that was located in the game files until recently.

Hat somewhat related to M. Bison (Dictator)?

/wiki hat/

It looks like a Dutch train-attendant's hat; with the railroad pin switched with a TF2 logo. FoOz 07:05, 5 October 2010 (UTC)

It appears to me to be a Peaked cap. They are used throughout many military forces. They orginated from Russia in the early 1700's. Jeff Andonuts 08:55, 10 October 2010 (UTC)

I think it looks like the SS hats of the 2nd World War, specially if you paint it black. I didn't like what's written on the "Distribution" section on this hats' page, it doesn't explain anything and it does not inform people if some players have already received the hat or not.
