Item sets are a game concept introduced in the Mann-Conomy Update, which applies new attributes (such as greater damage resistance, immunity to headshots, etc.) to the wearer when all items in a set are equipped. There are currently eight item sets, of which five originated from the Polycount Pack. Another three were later added in the Australian Christmas update, leaving the Engineer as the only class currently without his own item set. These item sets also do not require the corresponding hat, in order for the set bonus to activate.
Since Valve is of the opinion that game-changing effects should be available for every player within a reasonable amount of time, hats included in these sets are the only hats in the game to have guaranteed crafting blueprints (not including the Bonk Helm), requiring four Refined Metal and any weapon from the set in question the player wishes to craft the hat for.

- Primary - Crusader's Crossbow: Fires special bolts that heal teammates and deal damage to opponents based on distance traveled; the longer the distance, the larger the amount of healing or damage.
- Melee - Amputator: On taunt: heals all nearby teammates within a certain radius, who emit glowing circles similar to that of the Buff Banner or Batallion's Backup.
Bugs
- Due to how the bullet vulnerability is calculated, certain non-bullet weapons deal 10% more damage to a Gas Jockey's Gear Pyro. This includes the Syringe Gun, all melee weapons, the Chargin' Targe, and most taunt attacks.
