Item sets are a feature introduced in the Mann-Conomy Update, which applies new attributes to the wearer when all items in a set are equipped, such as greater damage resistance or immunity to headshots. There are currently eight item sets; five originated from the Polycount Pack, and another three were later added in the Australian Christmas update, leaving the Engineer as the only class to currently lack his own item set. The item sets introduced in the Australian Christmas update do not require the corresponding hat in order for the set bonus to activate.
Hats required for the item set bonuses that were introduced in the Polycount Pack were given guaranteed crafting blueprints. The Polycount set hats require four Refined Metals and any weapon from the set of which the player wishes to craft the hat.

- Primary - Crusader's Crossbow: Fires special bolts that heal teammates and deal damage to opponents based on distance traveled; the longer the distance, the larger the amount of healing or damage.
- Melee - Amputator: On taunt: heals all nearby teammates within a certain radius, who emit a teamcolored glowing circle effect similar to that of the Battalion's Backup.
Note: Only the two weapons are needed for the set bonus. The Berliner's Bucket Helm is not required.

- Added 'The Special Delivery', 'The Tank Buster', 'The Gas Jockey's Gear', 'The Croc-o-Style Kit', and 'The Saharan Spy' item sets.
December 17, 2010 Patch (Australian Christmas)
- Added 'The Medi-eval Medic', 'The Hibernating Bear', and 'The Expert's Ordnance' item sets.
- Fixed players getting set bonuses applied on tournament servers when the server is restricting the player's weapons using the item_whitelist.
- [Undocumented] Fixed players getting set bonuses applied on Medieval maps for items that are not allowed in medieval mode. This is presumably a side-effect of the previous fix.
Bugs
- Due to how the bullet vulnerability is calculated, certain non-bullet weapons deal 10% more damage to a Gas Jockey's Gear Pyro. This includes the Syringe Gun, the Chargin' Targe, and most taunt attacks.
- Snipers who have the entire Croc-O-Style Kit equipped can still die from headshots if they have around 1 health point. This is either a bug, or a misconception caused by the set bonus description.
