Item sets are a feature introduced in the Mann-Conomy Update, which applies new attributes to the wearer when all items in a set are equipped, effectively changing their class's role to a completely different one. Examples include increased damage resistance, an immunity to headshots, and silenced cloaking noises. There are currently sixteen item sets; the five Polycount Pack contest winners, three Australian Christmas sets, and eight Über Update sets.
The Polycount Pack item sets require players to have the set hat equipped to activate the item set bonus. The Australian Christmas item sets do not require the corresponding headgear to activate the set bonus. The Über Update sets, while they require their respective headgear and, if applicable, miscellaneous item to complete the set, do not bestow a set bonus. All set hats and misc. items can be crafted using four refined metal and a weapon from the set the desired hat/misc. item belongs to.
If a part of an item set is equipped, a list of items in the set, as well as which are currently equipped, appears when the item is inspected, either in the loadout menu or by other players. If the entire set is equipped, any effects it provides are also listed.

December 17, 2010 Patch (Australian Christmas)
- Added 'The Medi-eval Medic', 'The Hibernating Bear', and 'The Expert's Ordnance' item sets.
- Fixed players getting set bonuses applied on tournament servers when the server is restricting the player's weapons using the item_whitelist.
- [Undocumented] Fixed players getting set bonuses applied on Medieval maps for items that are not allowed in medieval mode. This is presumably a side-effect of the previous fix.
June 23, 2011 Patch (Über Update)
- Added 'Black Market Business', 'The Man of Honor', 'Lawrence of Australia', 'One Thousand and One Demoknights', 'The #1 Fan', 'The General's Formals', 'The Airborne Armaments', and 'The Clinical Trial' item sets.
Bugs
- Due to how the bullet vulnerability is calculated, certain non-bullet weapons deal 10% more damage to a Gas Jockey's Gear Pyro. This includes the Syringe Gun, the Chargin' Targe, and most taunt attacks.
- Snipers who have the entire Croc-O-Style Kit equipped can still die from headshots if they have 1 health point. This is either a bug, or a misconception caused by the set bonus description.
- Despite that the crafting screen will present the player with a "Fabricate Set Headgear" prompt when four refined metal and a weapon from the Hibernating Bear, Expert's Ordnance or Medieval Medic set is placed in the craft boxes, the Big Chief, Scotch Bonnet and Berliner's Bucket Helm all lack specific crafting recipes and cannot be obtained this way. This is because these hats are not required to obtain the bonus in their respective sets.

- Some of the sets were given different names by Valve than the original creators chose for them
- The Hibernating Bear was originally called Big Chief Hibernating Bear. [1]
- The Medieval Medic was originally named The Medi(c)eval Medic. [2]
- Black Market Business was originally called Black Market Heavy.
- The version of this pack with changes closer to the released version was called the Mobster Heavy.
- The Man of Honor was originally called The Bloody Valentine Pack.
- Lawrence of Australia was originally called The Australian of Arabia.
- Due to how damage resistance is calculated, equipping the Chargin' Targe with the Expert's Ordnance item set results in a total fire damage resistance of 55%. Equipping the Splendid Screen with the Expert's Ordnance will result in a total fire damage resistance of 28%.
- According to the creator, none of the items from the General's Formals set were submitted together as a pack, they were each submitted individually.
