Scream Fortress 2018 var en stor uppdatering som släpptes den 19 Oktober 2018. Med uppdateringen kom 2 nya hån, 5 nya halloweenkartors, 20 nya kosmetiska föremål skapade av gemenskapen, 4 nya säsongs-begränsade kosmetiska föremål skapade av gemenskapen, 10 nya stridsmålningar också skapade av gemenskapen, och 17 nya ovanliga effekter. Gratis Halloweenkontrakt återvände med ConTrackern för uppdateringen med 5 nya kontrakt tillagda för de nya Halloweenkartorna.
