アクションアイテムはゲーム内で使用できるアイテムです。Mann-Conomy アップデートで追加された要素で、工具アイテムと同じように使用回数に制限（バックパック内アイコンで表示されます）があるものが多く、最大の違いは（以前は "MISC2" と呼ばれていた）スロットに装備できるということです。ほとんどのアクションアイテムはMann Co.ストアで購入することによってのみ入手が可能ですが、決闘ミニゲームといくつかのタウント系アクションアイテムは、ランダムドロップやMann Co. 物資箱の開梱によって手に入ります。、現在、アクションアイテムはクラフトによって手に入れることはできません。

アクションキー（デフォルトではH）を押すと現在アクションスロットに装備しているアイテムを使用することができます。注意すべき点として、いくつかのアイテムはキーを押すと確認画面が表示されます。例えば決闘ミニゲームを使用した場合、決闘相手のプレイヤーを選択する画面が表示され、ギフト系のアイテムを使用すると本当にしようするかどうかの再確認画面が表示されます。しかしノイズメーカーといったアイテムは即座に使用されます。

What's in the Sandvich Box?		Contains a Robo-Sandvich and a Triple A Badge, both in Genuine quality when the box is opened.	Given to the Template:W attendants who redeemed the code distributed with purchasable replica Sandvich toys.	1
What's in the Companion Square Box?		Contains a Friends Forever Companion Square Badge and a Triple A Badge, both in Genuine quality when the box is opened.	Given to the Comic-Con San Diego 2012 attendants who redeemed the code distributed with purchasable Companion Square toys.	1
What's in the Portal 2 Soundtrack Box?		Contains an Ap-Sap in Genuine quality when the box is opened.	Given through a redeemable code inside the CD case of the Portal 2 Soundtrack.	1
Gift-Stuffed Stocking		Contains a Giftapult, Backpack Expander, Name Tag, Description Tag, Tour of Duty Ticket, and 2-3 random drops. All except the random drops are untradeable.	Given to players during the Smissmas 2013 update.	1
