"MEDIC!" is written in a style similar to that of a big band, with four sections of saxophones, trumpets, trombones, and rhythm, though with a reduced number of instruments in each section. Additionally, it contains orchestral harp and percussion, as well as a choir, which is heavily featured at both the beginning and end of the track.
The track begins with a choir introduction, accompanied by horn and strings, and accented by percussion. The choir resolves to a C power chord, as the rest of the ensemble enters. Tenor saxophone 2, tuba, electric guitar, and electric bass play the bass line outlining C minor, as tenor saxophone 3, trumpets, and trombones all play the melody line. This is then moved to F minor, then back to C minor, then a transition brings a new section omitting electric guitar. The same chord movement continues, into a complete repetition of the first section. After this, choir re-enters and ushers in a tenor saxophone solo which plays above the same melody as previous sections. In addition to C minor and F minor, the solo section moves to G minor as well, then back to C minor. This chord movement of i-iv-v-i could be seen as an altered version of a minor 12-bar-blues progression, which uses a similar chord structure. Finally, the end comes with harp glissando, a held C major chord from choir, hits from tuba, electric bass, guitar, and drums, and a screaming trumpet fanfare.
Lyrics
Official sheet music was posted on the TF2 Official Blog on July 10, 2020. Contained in the score for Medic! are the Latin lyrics heard in the beginning. They are:
- Nex est totus in ter
- Medicus comodo servo
- vel nos mo osto tus inter
- Ah...
This roughly translates to
- Death is all around but the doctor will keep all of us whole
