The Sandvich (in full, the Sandvich Edible Device) is an unlockable secondary 'Lunchbox' item for the Heavy, replacing his standard Shotgun once Heavy Milestone 1 is achieved. It consists of two triangular slices of bread containing lettuce, tomatoes, cheese, ham, and bologna in between. When eaten, it replenishes the Heavy to his full 100% health (300 HP) over a four second duration. The decision to eat a Sandvich must not be taken lightly however, as the Heavy will be completely immobile and creates loud munching noises during the process. After consumption, the Sandvich will be unavailable for a cooldown of 30 seconds, or until the Heavy picks up a Medikit while at full health. The healing given by the Sandvich does not extinguish fire although it heals the Heavy much faster than he is burned, potentially saving a Heavy with critically low HP. The Sandvich cannot be consumed whilst underwater (since it becomes wet and unsavory).

- The Sandvich has a community-created variant: The Dalokohs bar
- The "Sandvich" Edible Device is a product of Mann Co.
- A Heavy holding the Sandvich drops no ammo or metal when killed, leaving behind his Sandvich instead. An enemy Scout who kills a Heavy eating a Sandvich may collect his victim's luncheon to earn the "Dodgers 1, Giants 0" achievement, as well as to gain 75 HP. All other classes will heal 50 HP when they pick up a fallen Heavy's Sandvich.
- The Sandvich was first portrayed to the playing community in the Meet the Scout video, weeks before it was released as an in-game item.
- Because the Sandvich has no unique taunt, taunting with the Sandvich held has exactly the same effect as using its primary function, though the short delay between switching weapons and being able to fire them is ignored if a player taunts with the Sandvich (Default key "G") instead of using it with Primary fire. This method of bypassing the weapon switch delay can be used to minimize the time in which you are vulnerable to attack.
- When eaten, critical Sandviches do not receive any extra bonuses or boosts apart from the aesthetic critical effects.
- Despite the Sandvich being made of bread, meat, cheese, and vegetables, it can somehow be crafted into scrap metal.
- In some circumstances, eating the Sandvich continuously can make a Heavy almost invulnerable against some attacks due to its fast healing rate. It is even possible to get hit by uncharged Sniper Rifle headshots several times without dying.
- This was changed with the July 8, 2010 Patch, which made the Sandvich have a similar cooldown seen in Bonk! Atomic Punch, preventing from eating his Sandvich continuously.
- However, as of this edit on July 10, 2010, it is possible to circumvent this via the "use" command to make him switch to one of his other weapons; he does not heal while Sandvich is not out, however.
- This was changed with the July 8, 2010 Patch, which made the Sandvich have a similar cooldown seen in Bonk! Atomic Punch, preventing from eating his Sandvich continuously.
- A Heavy eating continuously his Sandvich can survive all the bullets shot by a level 1 sentry without dying.
- The Sandvich user guide says that the bread and grains group heals 20 health, the vegtable group heals 40 health, the cheese and dairy group heals 20 health, the meat and poultry group heals 20 health, and the other bread and grains group heals 20 health. This sums to only 120 health, the amount of health that the Sandvich originally replenished. The ability of the Sandvich to heal 300 points of health was added in a later update.
- It is also possible to drop the Sandvich possibly saving the Heavy's life while he is stunned from the Sandman.
- Currently, a glitch exists where if a player taunts with the Sandvich, The cooldown meter gets stuck and does not refill until you grab another Sandvich from the resupply cabinet.
