The Pain Train is a community-created melee weapon for the Soldier and the Demoman. It is a broken wooden handle bottom-wrapped with dark tape, outfitted with a large railroad stake and several bent nails stuck in the top.

When equipped, the weapon increases its wielder's capture rate by one, counting the wielder alone as two people and at par with the Scout when capturing control points and pushing Payload carts. As a downside, the wielder is 10% more vulnerable to bullet-firing weaponry such as the Minigun, Scattergun, Shotgun, Sniper Rifle, and Sentry Gun.

During a Critical hit, the Soldier swings it from the left in a backhand motion, while the Demoman swings it downwards in a swift motion.
