It's some form of self-aware beauty mark that only metastasizes in an environment of pure wheat.
The Medic

The Self-Aware Beauty Mark is a secondary weapon for the Sniper. It is a sealed jar containing translucent green fluid which sloshes inside the container as the player moves. There is a bread monster suspended inside it. A label on the jar reads "SPECIMEN; DO NOT OPEN" with the serial number of #384. The bread monster struggles and squirms inside the jar while growling.

This weapon functions identically to the Jarate, however, upon a direct hit on a target, a bread monster will appear squirming and biting the target's head on the side they were hit from for 10 seconds. If hit more than once, it is possible to accumulate several bread monsters on a single victim, which will additionally stack with any from Mutated Milk. If the target is killed, all of the bread monsters on them will persist on their corpse for 10 seconds, regardless of how long they have been sodden. This is a cosmetic effect and doesn't damage the player.

If the Self-Aware Beauty Mark is crit boosted when the projectile is thrown, the bread monster's size on targets it hits directly is drastically increased.
