Upon taunting with the Gunslinger equipped, the Engineer pulls a rip-cord from his mechanical prothesis, revving the hand of his Gunslinger up to vicious speeds, after which he thrusts it forward, shredding any enemy player slow, unaware, or simply unfortunate enough to be within reach at the time. The taunt inflicts several 1-damage hits and stuns the victim before inflicting a final blow which deals over 450 damage, instantly killing any class that isn't invulnerable at the time.
Like other taunts, the Organ Grinder can be performed during Humiliation as a last-ditch effort to defend oneself or to take out an enemy on the winning team barreling into the spawn room, as well during Stalemate.

- This taunt, along with the Medic's Spinal Tap, are the only taunt attacks without an accompanying achievement.
- If an enemy is killed by the hand's spinning they will ragdoll, but if they are killed by the removal of the hand, they will gib.
- This is the third taunt that has a stun effect before the attack intended to kill, the other two being the Medic's Spinal Tap and the Sniper's Skewer.
- This is one of only two taunts that can gib an opponent, the other one being the Soldier's Kamikaze (unless one counts the Demoman's Decapitation as gibbing).
- This is one of only 4 kill taunts in the game that is not completely silent up to the kill (Not including generic sounds like weapons clinking). This means that, unlike most kill taunts that can be done in secret, the Engineer has to give up his location in order to pull off this taunt, which is done by the Gunslinger's motor revving. (Other noisy kill taunts: Pyro's Hadouken, Spy's Fencing, and Engineer's Dischord.)
- Unlike most other taunt attacks, Organ Grinder does not hit based on the enemy player's position relative to the taunt animation- to hit, the target must be on the line indicated by the Engineer's crosshairs at the start of the taunt. As a result, the taunt can be aimed in any direction, including straight up or down. Showdown is the only other taunt that functions this way.
