The Skull Island Topper is an unlockable headwear item wearable by all classes. It is a ram's skull that is missing the left horn and has the right eye intact which wobbles around as the player moves. The Skull Island Topper is the hat worn by Merasmus. The hat tracks the highest level of the wizard that the player helped to defeat.
The hat can be obtained by earning the A Lovely Vacation Spot Necromannchievement by reaching the hat on Skull Island in Ghost Fort, after defeating Merasmus.

- The Skull Island Topper was added to the game.
