The Master Mind is a community-created headwear item for the Engineer. It is the Engineer's scalp with its skin ripped off exposing his brain, which has a strange mechanical apparatus attached to it. It was added in game for the 2012 Spectral Halloween Special.
The Master Mind was originally contributed to the Steam Workshop under the name "The Brainframe".
