The Skull Island Topper is an unlockable headwear item wearable by all classes. It is a ram's skull that is missing the left horn and has the right eye intact which wobbles around as the player moves. The Skull Island Topper is the hat worn by Merasmus.
The hat can be obtained by earning the A Lovely Vacation Spot Necromannchievement by reaching the hat on Skull Island in Ghost Fort, after defeating Merasmus.
