The Under Cover is a headwear item for the Spy. It is a wide-brimmed hat that sits atop a set of bandages that completely covers the Spy's head.
This hat can only be worn around Halloween or in a Full Moon, so it does not appear in gameplay. However, it can be equipped in the headgear loadout slot, and in the character preview it shows the player wearing the hat.
