The Medic Mech-Bag is a miscellaneous item for the Medic. It replaces the default backpack with a metallic backpack. It's the same backpack that the Medic's robotic counterpart wears. This item can be obtained by playing Mann vs. Machine (Mann Up Mode) with a Tour of Duty Ticket.
