The Familiar Fez is a headwear item for the Spy released with the Polycount Pack in the Mann-Conomy Update. It appears as a team-colored Fez and a pair of sunglasses that sit atop the Spy's regular balaclava.

As with all of the item set hats, this hat can be crafted from four Refined Metals and one weapon from the Saharan Spy set.
