The Second Opinion is a headwear item for the Medic. It gives the Medic a discolored skin graft with visible stitches. Equipping this item alters the Medic's voice responses.

Trivia
- A Second Opinion is when a patient requests to see a second physician, usually prior to a surgery, to get someone else's opinion.
- The item could be a reference to Two-Face, a recurring antagonist in the Batman series of comic books. In them, Two-Face is a man with half his face disfigured, and also mentally unstable. Both characteristics apply here, as seen by the Medic's new voice responses.
- This item could also refer to Dr. Jekyl and Mr. Hyde, a character from a book of the same name, who also is mentally unstable and has two sides of his personality. This is less likely, however, because the Mr. Hyde completely turns into Dr. Jeykl, and visa-versa, and are never both present at the same time.
- One of the responses is the reference to the ,,Shining" by Stephen King (The ,,evil side" will chant ,,Redrum" frantically). The ,,normal side" does not get it
