The Vintage Tyrolean is an unlockable headwear item for the Medic. It appears as a darkly team-colored felt cap with a braided cord band around the crown. To the left side of the hat, a formal-looking crest and a feather are added as trim, both of which are also team-colored. The narrow brim slopes downward towards the front, and is folded flat against the crown in the back. The crest consists of 2 gems and 2 gold(red team) or 2 silver(blu team) leaves. The 2 gems change color depending on which team you join.

Trivia
- Tyrolean is the name for the particular style of hats made in the County of Tyrol in the Austrian Alps.
- The Tyrolean hat is also known as the Tyrol and the Bavarian cap.
- The Tyrolean hat is one of the signature articles of clothing that is worn at Oktoberfest.
- Because the word "Vintage" was already in its name, any Vintage Tyrolean prior to the Polycount Update will now be named a "Vintage Vintage Tyrolean". This has given it the title of a "double-vintage" hat by the community. The only other hat that shares this title is the Vintage Merryweather.
