The Vintage Tyrolean is an unlockable headwear item for the Medic. It appears as a darkly team-colored felt cap with a braided cord band around the crown. To the left side of the hat, a formal-looking crest and a feather are added as trim, both of which are also team-colored. The narrow brim slopes downward towards the front, and is folded flat against the crown in the back. The crest consists of 2 gems and 2 gold(RED team) or 2 silver(BLU team) leaves. The 2 gems change color depending on which team you join.

- The Tyrolean hat is also known as the Tyrol and the Bavarian cap.
- The Tyrolean hat is one of the signature articles of clothing that is worn at Oktoberfest.
- The Pyro's Vintage Merryweather was originally submitted as the "Vintage Pyrolean" (most likely a play on the Vintage Tyrolean's name) and is labeled "pyrolean" in the game files.
- Due to pre-Mann-Conomy Update hats being given a "Vintage" status, it is possible to obtain a "Vintage" Vintage Tylolean.
