The Horseless Headless Horsemann's Head is a headwear item available to all classes. It is awarded to all players who earn the Sleepy Holl0WND achievement, by killing the Horseless Headless Horsemann that spawns on Mann Manor. It is available during the Halloween events only.

Trivia
- Despite covering the whole head, items such as the Camera Beard can still be worn with it equipped.
- There is a slight clipping problem when this hat is worn on Pyro. The gas mask's filter will poke out from the mouth of the hat.
- There is a slight clipping problem when this hat is worn on Scout. As he runs, and his dog tags fly behind him, they will poke through the back of the hat.
- Its level isn't randomly generated upon drop. It is always 31, presumably because Halloween is on the 31st of October.
- The eyes and mouth glow in the dark, like the bulb of the Mining Light or Earbuds.
- While wearing the Horseless Headless Horsemann's Head you are Immune to the Horseless Headless Horsemann's "Boo" Taunt.
