The Vintage Tyrolean is a headwear item for the Medic. It appears as a darkly team-colored felt Template:W with a braided cord band around the crown. To the left side of the hat, a formal-looking crest and a feather are added as trim, both of which are also team-colored. The narrow brim slopes downward towards the front, and is folded flat against the crown in the back.

Trivia
- Tyrolean is the name for the particular style of hats made in the County of Tyrol in the Austrian Alps.
- The Tyrolean hat is also known as the Tyrol and the Bavarian cap. It is one of the signature articles of clothing that is worn at Oktoberfest.
- When the MannConomy update arrived, the name had the prefix "vintage" added on. Resulting in the vintage version reading "Vintage Vintage Tyrolean". This is yet to be fixed by VALVe.
- The Tyrolean bares an uncanny resemblance to the Fancy Fedora except for the feather.
