The Quadwrangler is a community-created promotional cosmetic item for the Medic. It is based on the Inter-Dimensional Shift Device and clothes used by Professor Fitz Quadwrangle from the video game Quantum Conundrum; a device attached to the back of the Medic's right hand with three team-colored and glowing vials equipped with a floppy tube that flows to a plug embedded in his upper arm, and a large cowl-like collar. It also changes his undershirt and tie to a team-colored turtleneck sweater.
This item was awarded in Genuine quality to players who pre-purchased Quantum Conundrum on Steam before June 21, 2012.
