It appears to be bolted down to a thin metal plate with 2 thin yellow wires wrapping around the top of the hat. The top appears to be fastened down with the rest of the hat by a little hinge.

The Virus Doctor is a community-created cosmetic item for the Medic. It is a robotic version of the Grimm Hatte.

The Virus Doctor was contributed to the Steam Workshop.
