The Spine-Chilling Skull is a headwear item available to all classes. It appears as a sharp-toothed, demonic skull with curled ram horns. It can be purchased from the Mann Co. Store only during the Scream Fortress event, running between the 27th of October and the 8th of November 2010. It also has a 1 in 2 chance of being crafted if an Unusual Haunted Metal Scrap is used alongside 4 Refined Metal (the other possible result is a Voodoo Juju).

Trivia
- Like several other Halloween hats, the Spine-Chilling Skull does not have a randomly selected level when crafted or purchased. It's level is always 31, presumably because Halloween is on the 31st of October.
- According to Valve, this hat was modeled by the same man that made Gollum's face in the Template:W. It also bears a remarkable resemblance to a Template:W, a creature in the first Lord of the Rings film.
- Most methods of obtaining this hat produce a non-tradable version. All store-purchased wearables are automatically not tradable or craftable, if you opt to craft the hat using the Unusual Haunted Metal Scrap, unlike most hats crafted with untradable metal the hat will become tradeable when crafted. However, if you craft it with an unusual metal that has been Gift Wrapped, it will become tradeable after a few days. The glitch to obtain non-tradable items through Gift Wrap has since been patched.
- The skull's teeth resemble those of Blutarch Mann as depicted in the background of Redmond Mann's portrait in Mann Manor.
