The Tsarboosh is a community-created cosmetic item for the Heavy. It is a little team-colored fez that is placed onto the Heavy's head, and has a small floppy tassel attached.
The Tsarboosh was contributed to the Steam Workshop under the name "The Unfamiliar Tarboosh".

- The name of this item is a play on the words "tsar", a title used to designate certain European Slavic monarchs or supreme rulers, and "tarboosh", the Arabic name for a fez.
