The Sammy Cap is a community-created cosmetic item for the Heavy. It is a full Sandvich suspended in front of the Heavy's face by a rope attached to a bent stick which is connected to a paintable headband worn by the Heavy.
The Sammy Cap was contributed to the Steam Workshop under the name "SammyCap V5 (Simplified Bones)".
